SUMMARY OF DUTIES
Provides short-term outreach and home visiting services to at-risk families. Engages families in services. Conducts intake/assessment with clients. Engages clients in development of service plan and goals. Teaches and provides information on a wide variety of topics, including child development, stress management, parenting, nurturing and family life as indicated by client needs. Provides linkage to community resources. Assists families, through advocacy or concrete help, to negotiate systems to receive needed services. Provides transportation to medical, legal and social service appointments as needed and after consultation with supervisor. Performs administrative tasks related to caseload such as progress notes, service plans, mileage and miscellaneous reports in a timely manner. Maintains client confidentiality at all times.
EDUCATION AND TRAINING REQUIREMENTS
- Four-Year College Degree from a school accredited by a recognized accrediting agency - general major. Requires basic knowledge or training in a specialized or technical field or trade, such as social work, professional accounting, finance, business administration, and marketing.
- Other (Specify): Bachelor’s Degree may be waived with over 5 years of relevant experience.
EXPERIENCE
- Over two years, up to and including four years.
- Other (Specify): Experience working with at risk families preferred. Case management experience preferred.
SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
- Good written and verbal communication skills.
- Ability to work with families of diverse ethnic, cultural, and socio-economic backgrounds.
- Ability to work flexible hours, including evenings and weekends as required.
OTHER POSITION REQUIREMENTS
- This job requires a valid Hawaii driver's license, a clear driver's abstract, and use of personal vehicle to transport clients, as needed, as well as a willingness to travel in personal vehicle on an as-needed basis throughout the island.
