Responsibilities
- Provide services where they are most effective—meeting clients in their homes or other community settings. You will offer initial triage and support during crisis situations, ensuring a rapid and grounded response.
- Utilize the IM-CANS assessment tool to co-create consumer-driven, personalized care plans. You’ll ensure all identified services meet program requirements while serving as a roadmap for the client's specific recovery goals.
- Conduct individual, family, and group therapy sessions using Evidence-Based Practices (EBPs). Your focus will be on education and skill-building to enhance client autonomy and daily living capabilities.
- Act as a dedicated advocate by providing case management, medication training, and consistent monitoring. You will help clients navigate the complexities of the healthcare system and understand their treatment regimens.
- Manage the essential "behind-the-scenes" continuity of care, including the pre-authorization and re-authorization of services to ensure treatment remains uninterrupted.

- Candidates for this position must possess a high school diploma or equivalent and have five years of supervised mental health experience.
- Alternatively, a bachelor’s degree in counseling, social work, education, psychology, or a related human service field is required.
- Individuals with a bachelor’s degree in any other field must have at least 2 years of supervised clinical experience in a mental health setting.
- Demonstrated skills in human services and in services to adults.
- Demonstrated ability to work within organizational structure; to accept supervision; and to work constructively with consumers, treatment resources, and the community.
- Basic computer skills including MS Word and an electronic medical record.
- Valid driver’s license, private auto insurance, and be insurable.
