What are the responsibilities and job description for the RN, Clinical Navigator position at Chesapeake Hospital Authority?

The Clinical Navigator acts as an advocate to assist patients in navigating throughout internal and external systems in order to facilitate the diagnosis and treatment process. Applies the nursing process through assessment, planning, implementation and evaluation of the ptient in order to provide the optimal level of patient care.

Essential Duties and Responsibilities

  • Exhibits excellent customer relations to patients, visitors, physicians, and co-workers.
  • Demonstrates courtesy, compassion, and respect to others in adherence of the hospital's philosophy and policy for promoting a positive work and customer environment.
  • Works closely with the team and physicians when performing interventional procedures
  • Works as a member of a quality team to continuously improve quality outcomes within the breast imaging. Supports radiology services by providing optimal patient care as allowable to Special Procedure, and the other multiple radiology services, as needed; CT, MRI, Diagnostic, Ultrasound, and Mammography. Additionally, responds to emergencies, hard sticks and contrast reactions within the department when not caring for a patient.
  • Fax reports to the referring physician's office to obtain a timely response for follow up biopsy to be
  • Manages all birad O patients, contacts the patient to education about additional imaging and makes follow up appointment.
  • Schedules and manages all breast biopsies for maximum
  • Responsible for providing optimum patient-care to radiology patients; also functions as a patient advocate. Assesses and monitors patient condition and initiates appropriate nursing

  • Coordinates effective handling of emergencies arising from contrast media reactions, seizures, cardiac arrest, etc.
  • Assists the Radiologist as a team member during interventional or diagnostic procedures within Mammography, Ultrasound and MRI biopsies.
  • Administers medications, changes dressings/special appliances, and monitors vital signs when
  • Acts as a liaison between Radiology, patient care units, and patient care
  • Provides instruction and education in IV therapy to technologists and completes annual recertifications of IV injections on technologists.
  • Performs daily AED checks. Conducts monthly code drills with
  • Responsible for tracking, entering data and reporting statistics through the National Mammography Database.
  • Meets with birad 4 I 5 patients providing them with education regarding results and next
  • Works closely with the Radiology RN to hand off and communicate regarding positive breast cancer patients and coordination of care.
  • Provides in-services in BLS, patient care, safety and infection control. Coordinates BLS classes for ancillary departments. Provides in services for technical staff. Assists with the orientation of nurses, physicians, technologists, and students.
  • Assists with quality improvement and risk management programs and participates in the department's quality improvement program. Performs necessary follow-up for concerns/incident
  • Works with the Director of Jennings Outpatient Center to review, revise, and develop policies and procedures relevant to patient care.

Education and Experience

Minimum Required Education: Registered Nurse licensed by the Virginia Board of Nursing OR licensure with the American Registry of Radiologic Technologists (ARRT), with advanced registry in Mammography.

Preferred Education: Associate's degree in a healthcare field with courses in Medical/Healthcare Terminology.

Experience: Must have at least 5 years of nursing experience or 5 years of breast imaging experience.

Certificates, Licenses, Registration

  • BLS required.

  • Certification in clinical breast examinations must be obtained within 1 year.
