Role: Speech Language Pathologist providing in-home, senior living, and virtual care to older adults in Londonberry, NH.
Responsibilities: Conduct evaluations, develop individualized care plans, and deliver evidence-based therapy to enhance patient wellness, mobility, and independence. Document accurately in EMR, collaborate with interdisciplinary teams, and educate patients and caregivers on safety and aging-in-place. Incorporate telehealth and virtual care to increase access and flexibility.
Qualifications & Skills: Valid state license or eligibility, degree from an accredited program, current CPR and ASHA CCC, computer proficiency, and physical ability to perform in-home care tasks including lifting, transfers, and driving.
Key Attributes: Autonomy, strong interpersonal skills, ability to build patient relationships, and physical endurance for community-based care. Experience with in-home or senior care settings is preferred.
