- Provide personalized, one-on-one physical therapy to older adults in private homes or senior living settings.
- Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapies to enhance mobility, independence, and quality of life.
- Document care effectively using electronic medical records and collaborate with interdisciplinary teams including PTs, OTs, SLPs, and fitness specialists.
- Educate patients, caregivers, and staff to promote safety and aging-in-place strategies.
- Utilize telehealth and virtual care options to increase flexibility and patient access.
- Valid PT license or eligibility in the practice state, with a degree from an accredited program.
- Proficiency in basic computer use and EMR systems, current CPR certification.
- Ability to meet physical demands such as lifting, repositioning patients, standing for long periods, and driving to patient locations.
- Autonomous, flexible, and dedicated to meaningful patient outcomes.
- Ability to create and adjust treatment schedules and incorporate virtual care modalities.
- Strong communication skills and commitment to fostering positive patient relationships.
