Role: Licensed Practical Nurse (LPN) providing skilled nursing care in patients' homes, following physician orders and agency protocols.
Responsibilities: Monitor patient conditions, administer medications, counsel patients and families, document treatments, and communicate changes to the RN. Participate in case conferences, on-call duties, and training sessions. Maintain professionalism and adhere to agency policies.
Qualifications & Skills: Current LPN license in the state, minimum one year acute care experience (two preferred), effective communication, compassionate nature, attention to detail, teamwork, and a valid driver’s license with insurance. Home care experience is desirable.
Key Duties: Drive to patient locations, perform visits, document within 24 hours, and support patient education to reduce re-hospitalizations. Ensure compliance with federal and state guidelines.
Physical Demands: Ability to drive, walk, lift up to 50 lbs., and perform various mobility tasks, with accommodations available for disabilities. Strong communication, computer usage, and observation skills are essential.
