- Communicating with physicians, team members, and community resources for coordinated patient care
- Documenting services accurately
- Understanding Medicare/Medicaid regulations for home health benefits
- Reporting patient condition changes to the RN
- Current Louisiana LPN license
- At least 1 year of clinical LPN experience
- CPR certification
- Compassionate with strong time management skills
