Hospice Social Worker

We are seeking a Social Worker to join a hospice transdisciplinary team, providing

supportive services to address patients' and families' social, emotional, and financial needs

related to terminal illness, loss, and grief.

Key Responsibilities Include

• Assessing social, emotional, and environmental factors affecting coping and safety
• Providing individual and family counseling on psychosocial aspects of end-of-life care
• Evaluating caregiver capacity and resource availability
  
  

Qualifications Include

• Master’s degree in social work from an accredited program
• At least one year of social work experience in healthcare, hospice experience preferred
• Valid California Driver’s License and insurance if driving
  
  

The role requires strong assessment, counseling, and communication skills to support patients and families through end-of-life challenges in a compassionate manner.
