Full-time Physical Therapist (PT) role in the Sebring FL area, working Monday through Friday in home health care.

- Conduct initial and ongoing assessments of patients' mobility, strength, and functional abilities.
- Administer, evaluate, and interpret tests to establish goals and measure outcomes.
- Develop and document treatment plans, including goals, progress, and discharge plans.
- Provide physical therapy services according to physician orders, ensuring proper documentation to avoid denials.
- Collaborate with healthcare team, patients, and caregivers; educate on home exercise plans.
- Supervise personnel such as PT Assistants and Home Health Aides where applicable.
- Complete required assessments like OASIS as permitted by state regulations.
- Ensure compliance with policies, standards, licensure, and laws.
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license as a Physical Therapist.
- Valid CPR certification.
- Preferred: strong organization, communication, attention to detail, time management, and experience with home health therapy and OASIS documentation.
