For individuals transitioning from closed custody correctional facilities, Cherry Health provides Person Centered Reentry Services to ease the transition back into the community. Services are available at two locations: our Community Treatment Center (CTC) in Detroit and Community Alternatives Program (CAP) in Grand Rapids. Services include 24-hour residential treatment in a structured environment with individual and group counseling services. Some participants come to us directly from a correctional facility, while others come as part of a diversion program to keep them out of prison. Together, we work to address health issues (including behavioral health issues) and other reentry needs i.e., employment, education, housing, and fostering connections to help justice system involved persons successfully reintegrate back into their respective communities.
The Clinical Coordinator - Reentry is responsible for planning, organizing, developing and monitoring quality assurance activities in a residential reentry setting.
Coordinate clinical training, staff scheduling and staff development. Provide assessment and treatment for a caseload of assigned residents.
Must adhere to the organizational Policies, Procedures, Code of Ethics, and all licensing and accreditation standards of the position.
Must represent the organization’s Mission in all actions and communications.
This is a full time, benefit eligible position typically working Monday - Friday onsite at our Community Alternatives Program.
Essential Duties And Responsibilities
Bachelor's degree in social work, psychology, counseling or related human service field required. CADC/CAADC or MCBAP development plan eligible required.

Essential Duties And Responsibilities
- Audit resident charts for clinical proficiency.
- Participate in the Chart Review Committee meetings.
- Ensure contractual obligations regarding productivity and frequency of resident contact are consistently met.
- Participate and present case conferences, training requirements and supervisory evaluation.
- Assist with monitoring of referrals, in accordance with program guidelines.
- Review and sign all discharge summaries.
- Train staff on current clinical and FBOP expectations of care.
- Assist supervisor with staff schedules of work, priorities and assignments to ensure efficient operation.
- Coordinate with community, referral and funding sources to provided needed services.
- Maintain current authorizations for service and complete utilization reviews as required by funding sources.
- Work with Cherry Health’s billing and finance departments to resolve authorization or payment issues for residents.
- Engage in PRT, work with interdisciplinary team including FBOP and US Probation Office representatives to assess resident progress.
- Complete and monitor monthly reports.
- Facilitate resident intakes.
- Provide resident assessments, treatment plans, individual and group sessions.
- Maintain timely clinical documentation and reporting within all standards and agency policies which apply.
- Communicate regularly and thoroughly with the clinical team.
- Become familiar with and maintain standards of FBOP guidelines as outlined in the FBOP Statement of Works.
- Maintain a caseload, including CTS aftercare programming.
- Adhere to compliance standards as they appear in Cherry Health’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy.
- Keep abreast of all pertinent federal, state and Cherry Health regulations, laws and policies as they presently exist and as they change or are modified.
- Perform other duties as assigned.
- Must be able to demonstrate knowledge of criminogenic factors, current addition theories and variety of cognitive behavioral therapies.
- Demonstrated knowledge of and ability to use methods of service planning and delivery with the client population served by the agency.
- Ability to communicate clearly and effectively, both orally and in writing.
- Have an understanding, and a working knowledge, of the person-centered planning process.
- Demonstrated ability to engage clients in service.
- Demonstrated ability to coordinate services with other community and client’s net resources.
- Must have valid Michigan driver’s license.

- Minimum 3 years experience in case management, biopsychosocial assessment, and treatment planning.
- Previous mental health and/or substance abuse and treatment experience strongly preferred.
