Case Manager
Chateau Health & Wellness | Residential Treatment
About Chateau Health & Wellness
Chateau Health & Wellness is a residential treatment program specializing in helping first responders, veterans, and adults struggling with trauma, mental health, and substance use challenges. We provide a highly individualized treatment experience designed to address the unique needs of a more mature population.
At Chateau, we believe treatment is about more than what happens while a client is in our program. Our goal is to help each client build a foundation for lasting change and leave treatment with a clear, realistic plan for continued success.
Position Overview
The Case Manager plays an important role in helping clients successfully navigate their treatment experience and prepare for life after residential care.
As a member of the multidisciplinary treatment team, the Case Manager works closely with clients, therapists, medical providers, families, referral partners, and outside service providers to identify needs, establish goals, and coordinate the resources and services necessary for a successful transition from residential treatment.
This is a highly relational and client-focused position. The Case Manager develops meaningful relationships with clients, meets with them regularly, helps them work through practical barriers, and ensures that the details of their discharge plan are thoughtfully addressed before they leave Chateau.
The ideal candidate is organized, compassionate, resourceful, and comfortable working with clients who may be navigating complex mental health, substance use, family, employment, financial, legal, or other life circumstances.

Discharge & Aftercare Planning
- Work collaboratively with clients and the treatment team to develop individualized discharge and aftercare plans.
- Identify appropriate continuing-care services and resources based on each client's individual needs and circumstances.
- Coordinate appointments and services prior to discharge, including outpatient treatment, therapy, psychiatry, support groups, medical care, and other community resources.

- Identify potential gaps in a client's support system and work proactively to address them before discharge.
- Communicate relevant discharge planning information to the treatment team and other appropriate parties.
Care Coordination
- Coordinate with clinical, medical, nursing, admissions, and other Chateau staff to ensure continuity of care.
- Communicate with outside providers, family members, employers, referral sources, and other authorized parties as appropriate.
