Our mission is to provide compassionate support to those in need who face behavioral health, co-occurring disorders or intellectual/development disability (IDD) challenges as they transform our lives.
As a Blended Service Coordinator with Chartiers Center, you would provide support and service coordination to adults living with serious mental illness and co-occurring conditions, including substance use disorders, physical health concerns and intellectual disabilities.
This is a highly rewarding community-based role where you build meaningful relationships, advocate for individuals and help them achieve greater stability, independence, and recovery.
Job duties include:
- Meeting with individuals in homes and community settings.
- Conduct assessments and identify strengths and needs and goals.
- Develop, implement and monitor individualized service plans.
- Connect individuals with housing, benefits, healthcare, behavioral health treatment, employment and community resources.
- Coordinate services among healthcare providers, therapist, family members and community agencies.
- Maintain regular communication with individuals to monitor progress and identify emerging needs.
- Advocate on behalf of individuals across multiple systems.
- Maintain timely and accurate documentation.
- Participate in an on-call rotation, typically no more than once per month.

Minimum Qualifications:
- Associate’s degree in human services and 3 years of mental health experience.
- Bachelor's degree in human services and 2 years of mental health experience OR
- Master’s degree in human services with one year of experience
- Valid Driver’s License Required
- Ability to travel throughout the community
