We hope you'll join our teamThe OpportunityChaparral Medical Group is seeking passionate and team-oriented Nurse Practitioners (NPs) and Physician Associates (PAs) to join our fast-growing, mission-driven care team. Based in our family of primary care clinics in the Pomona Valley region, you'll play a key role in delivering high-quality, compassionate care to adult patients across a broad range of needs. You'll work alongside a collaborative team of clinicians, benefit from integrated virtual support, and be empowered to make a real impact on the health of our communities.What You'll DoManage a patient panel, delivering care across acute, chronic, and preventive visitsProvide in-person care at a clinic site with occasional telehealth appointments as neededCollaborate with in-office teammates during daily huddles and virtual clinical team check-insPerform in-office procedures aligned with your clinical training and credentialsSupport patients in their care journey by coordinating referrals and follow-ups with specialists and facilitiesMaintain accurate and timely patient documentation within our electronic medical records systemContribute to a team culture of quality, collaboration, and continuous improvementWho You AreCompleted an accredited NP or PA program with current national certification (required)Licensed to practice in the state of California (required)In the past 5 years, you have: 2 years of experience as an Advanced Practitioner in an outpatient primary care setting, OR1 year in outpatient primary care and either 1 year in a primary care fellowship or 1 year in urgent care (required)Strong diagnostic and clinical decision-making skillsExcellent communication, patient education, and interpersonal abilitiesComfortable working in a team-based environment and adapting to a fast-paced clinic settingPlease Note: While you will work at a Chaparral Medical Group (CMG) clinical site, your employer will be Akido MSO LLC, which shares common ownership with CMG.Salary range$130,000—$150,000 USDChaparral Medical Group and Akido MSO are an equal opportunity employers, and we encourage qualified applicants of every background, ability, and life experience to contact us about appropriate employment opportunities.
