Resident Care Coordinator:

$26-28/hr

Channel Point Village is seeking a Resident Care Coordinator to join our team! Grow your skills with our team of compassionate individuals who love what they do and who they serve!

Under the direction of the Director of Nursing and the Executive Director, the Resident Care Coordinator provides an overall management supervision to the staff and program for the Assisted Living Department.

RESPONSIBILITIES:

• Supervision and performance monitoring of team members in Assisted Living and the application of department services to residents in the community.
• Provide caregiving coverage to ensure resident care is not impacted in the event of staffing shortage.
• Prompt notification to ED, Director of Nursing, or Wellness Nurse of all emergencies and situations beyond the scope of your capabilities.
• Participation in resident orientation, and identification of potential changes or concerns within the care plan of each resident.
• Participate in required training, meetings and orientations offered by employer
• Encourage positive relations and involvement with family members of Assisted Living residents, including attending and participating in scheduled Family Care Conferences.
• Supervise the use of the 24 Hour Report Log by caregivers including monitoring MD fax responses, completion of Incident Reports and Alert Charting information.
• Create and post monthly schedules for the Assisted Living caregivers and assign resident care daily.
• Monitor completion of monthly Vital Signs and weights and communicate with the nursing department any concerns related to them.
• Attend all care staff evaluations and counseling sessions.
• Provide support for other designated areas/units as needed.
• Perform other duties as assigned.

QUALIFICATIONS:

• Read, write and speak and understand the English language
• Freedom from illegal use of drugs
• Freedom from use and effects of use of drugs and alcohol in the workplace
• Persons who have been found guilty by a court of law abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment in the position.
• Demonstrates effective time management skills.
• Ability to pass DSHS background check, required.
• Must be over 18 years of age.
• Caregiving in Assisted Living with a minimum of 4 years’ experience.
• Management/Leadership skills of preferably 2 years’ experience.
• Scheduling experience of preferably 1 years.

This is a Monday- Friday, day shift position and availability to be on-call is required.
