Responsibilities
- Provide comprehensive nursing assessments, including vital signs, physical examinations, and patient history documentation using Electronic Health Record (EHR) systems.
- Administer medications, injections, IV infusions, and perform catheter care following strict protocols to ensure patient safety and comfort.
- Develop and implement tailored care plans for patients with various needs such as dementia, Alzheimer's disease, chronic illnesses, or post-acute recovery.
- Educate patients and their families on disease management, medication usage, nutrition, and health coaching to promote optimal health outcomes.
- Coordinate with interdisciplinary teams for discharge planning, case management, and referrals to community resources or specialty services like hospice or physical therapy.
- Conduct patient assessments during home visits, monitor symptoms of acute pain or infectious diseases, and triage urgent concerns with professionalism and urgency.
- Maintain accurate medical documentation adhering to HIPAA regulations while utilizing EMR/EHR systems such as Epic or Cerner for seamless record keeping.
Skills
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Bilingual proficiency in [specify languages], enabling effective communication with diverse patient populations.
- Critical care experience including ICU or emergency medicine settings is highly desirable.
