Care Manager – CVFC
Champlain Valley Family Center (CVFC) is seeking a dedicated Care Manager to support individuals with complex behavioral health needs. This position will play a key role in bridging care between clients and community-based supports, ensuring individuals receive the resources and follow-up care they need for improved well-being.
Key Responsibilities:
- Providing direct services to people with serious mental illness, developmental disabilities, alcoholism or substance abuse
- Coordinate care across medical, behavioral health, and social service providers to support long-term stability.
- Provide hands-on assistance with housing, benefits, transportation, and other essential services.
- Advocate for individuals and work collaboratively with hospital staff, community agencies, and peer support networks.
- Utilize lived experience and/or professional background in human services to build rapport and empower individuals in their recovery journey.
Qualifications:
- Bachelor’s degree in Human Services, Social Work, or a related field (or equivalent experience) with 2 years experience, or Master's with one year of experience, or a CASAC and 2 years experience
- Lived experience with mental health, substance use recovery, or navigating social services is highly valued.
- Strong communication, problem-solving, and advocacy skills.
- Ability to work collaboratively across hospital and community settings.
- Knowledge of local resources and experience with case management preferred.
