Clinical Liaison Nurse
Champion Care is seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our growing team as a Clinical Liaison Nurse supporting our skilled nursing and rehabilitation communities throughout the Birmingham area.
This is an excellent opportunity for a relationship-driven nurse who enjoys collaborating with hospitals, physicians, case managers, discharge planners, and healthcare professionals while helping patients transition to high-quality post-acute care. Your clinical expertise and communication skills will play a key role in developing referral relationships, coordinating admissions, and ensuring outstanding patient outcomes.
Responsibilities
Build and maintain strong relationships with hospitals, physicians, discharge planners, case managers, social workers, and other referral sources.
Serve as the primary clinical liaison between hospitals, healthcare providers, and Champion Care's skilled nursing and rehabilitation communities.
Review patient referrals and evaluate clinical appropriateness for admission.
Coordinate smooth transitions from hospitals and other healthcare settings into skilled nursing and rehabilitation facilities.
Collaborate with admissions teams, nursing leadership, therapy departments, and interdisciplinary care teams.
Educate referral partners on Champion Care's skilled nursing, rehabilitation, long-term care, memory care, and specialty services.
Represent Champion Care through community outreach, hospital visits, networking events, and relationship-building activities.
Monitor referral activity and maintain timely communication with referral sources.
Identify opportunities to increase referrals and strengthen strategic partnerships.
Resolve barriers to admission while ensuring exceptional service for patients and families.
Travel throughout the assigned territory as needed.
Qualifications
Current Alabama Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
Experience in skilled nursing, rehabilitation, hospital nursing, admissions, case management, discharge planning, care coordination, clinical liaison, or healthcare business development preferred.
Strong understanding of Medicare, post-acute care, skilled nursing, rehabilitation services, and hospital discharge processes.
Excellent communication, presentation, relationship-building, and organizational skills.
Self-motivated with the ability to work independently and manage multiple priorities.
