Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Willow Haven Healthcare & Rehabilitation. In this role, you will provide high-quality nursing care to our residents, ensuring their health and well-being while contributing to our mission of family-focused care.

This position has a starting pay rate of $28.50 an hour. As part of the Weekend Warrior program work 24 hours and get paid for 32 hours.

This schedule is every Friday and Saturday from 7pm-7am.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Monitor patient health and provide updates to the healthcare team.
• Assist in the development and implementation of individualized care plans.
• Perform assessments, including vital signs and medical history evaluations.
• Educate patients and families on health management and wellness practices.
• Coordinate with interdisciplinary teams to ensure comprehensive care delivery.
• Document patient progress accurately in electronic health records.

Requirements

• Current Licensed Practical Nurse (LPN) in the state of Ohio.
• Experience in Long Term Care, Behavioral Health or Acute Care preferred.
• Strong clinical assessment skills and attention to detail.
• Excellent communication and interpersonal skills for effective patient interaction.
• Familiarity with electronic health record systems such as Point Click Care is a plus
