Job Description:
Under the supervision of the Nurse, the Medical Coordinator handles administrative tasks. The role of the Medical Coordinator is to make sure DSPs are aware of the responsibilities associated with caring for individuals’ health and help with those tasks. The coordinator will monitor CMTs and ensure medication administration is done in the residential homes at the proper times, schedule and attend medical appointments, manage medications, and follow up with care providers about individual conditions/ health needs.
Duties and Responsibilities:
· Communicating between staff, residents, and family.
· Handling residents medical case management and record keeping.
· Maintain the medical inventory to include prescriptions, vitamins and first aid.
· Developing plans for medical goals and monitoring progress.
· Accompanying residents to medical appointments & emergency room visits.
· Verifying staff prescription distribution procedures.
· Communication to House Nurse.
· Maintaining confidentiality and quality care.
License:
·CMT Certification (Required)
·DDA Certifications (Required)
·CPR First Aide Certification (Required)
·Driver's License (Required)
Education & Experience:
· High School Diploma or GED required.
· Minimum 2 years of experience working in a residential facility.
· Minimum 2 years of management/ leadership experience in a residential facility.
· Good written and verbal communication skills.
· Strong desire to help others in need and willingness to learn new skills necessary for the job.
· Demonstrated ability to work effectively and collaboratively with staff, supervisors, other care providers, family, and community members.
· Possession of a valid driver’s license, proof of current insurance, and a clean DMV record required. Possession of a reliable car.
Schedule:
· Monday to Friday
· On call
· Weekend availability
