Summer Transition Instructor

JOB SUMMARY: To provide engaging activities to teens participating in the Summer Program.

DUTIES AND RESPONSIBLITIES:

1. Instruct and implement all program activities, including swimming and community field trips.
2. Assist with activity planning as appropriate.
3. Maintain communication with supervisor and co-workers.
4. Inform supervisor of any client and staff concerns.
5. Communicate with parents and caregivers regarding client’s daily activities.
6. Provide assistance with personal cares for male and female clients, such as, toileting, feeding, dressing, showering, transportation on and off the bus
7. Complete necessary documentation to include: client notebooks, attendance, special health care procedures, incident reports, worker’s compensation form, seizure reports, behavior reports, etc.
8. Attend staff meetings and trainings as required.
9. Provide direction to volunteers during program activities and community outings
10. Clean up program areas after use as needed and maintain a safe work environment.
11. Prepared to assume responsibilities as appropriate and assigned by supervisor.
12. Adhere to the agency safety policies and procedures.
13. Adheres to Drug Free Workplace policy.

EXPERIENCE AND SKILL REQUIREMENTS:

1. Experience and/or training working with kids/adults who have severe and multiple disabilities.
2. Physically able to lift and transfer individuals (minimum of 25lbs).
3. Must have the balance and coordination to assist clients with ADL tasks.
4. Successful completion of background check, lift test, communicable disease screen and drug screen.
