Registered Nurse Case Manager
Location: Barre, Vermont
Pay Rate: $2,300 To $2,560/Week
Contract: 12 Weeks
Schedule: Day Shift, Monday Friday
Experience: Minimum 1 year of Home Health RN experience required
About the Role
We are seeking a compassionate Home Health Registered Nurse (RN) for a 12-week contract opportunity with Central Vermont Home Health & Hospice in Barre, Vermont. This role is ideal for a dedicated nurse who is passionate about delivering individualized home health care, coordinating patient services, and serving as a Case Manager to support patients in achieving the best possible outcomes in their homes.
Responsibilities
Provide skilled nursing care and comprehensive patient assessments in the home setting.
Develop, implement, and evaluate individualized care plans based on physician orders.
Serve as the primary Case Manager and coordinate care with physicians and the interdisciplinary team.
Educate patients and caregivers on disease management, medications, and home care needs.
Document patient care accurately using Homecare Homebase (HCHB).
Requirements
Active Vermont RN License required.
Minimum 1 year of Home Health RN experience.
Current BLS/BCLS Certification required.
Experience with Homecare Homebase (HCHB) or similar home health EMR preferred.
Strong case management, assessment, and patient education skills.
