About the RoleWe are seeking a Physical Therapist (PT) – Skilled Nursing Facility (SNF) for a 13-week contract in Hayward, California. This position provides physical therapy services in a skilled nursing setting with 30 guaranteed hours per week.
Candidates must be available to work either a Sunday–Thursday or Tuesday–Saturday schedule.
ResponsibilitiesProvide physical therapy services within a Skilled Nursing Facility (SNF) setting.
Deliver rehabilitation services to assigned skilled nursing patients.
Support patients in achieving functional rehabilitation goals.
Maintain appropriate physical therapy documentation for assigned patients.
Collaborate with the rehabilitation team to support continuity of patient care.
Current, Active Physical Therapist License Required; Specific State Licensure Requirement Must Be Verified.
Less Than 1 Year of Physical Therapist Experience Accepted Per Requisition.
Ability to Work in a Skilled Nursing Facility (SNF) Setting Required.
Availability for a Sunday–Thursday or Tuesday–Saturday Schedule Required.
Ability to Meet a 30-Hour-Per-Week Guaranteed Schedule Required.
