About the RoleWe are seeking an experienced Physical Therapist (PT) Home Health for a 13-week contract in Lewistown, Montana. The Physical Therapist will provide skilled therapy services to Home Health and Hospice patients, completing comprehensive evaluations, developing individualized treatment plans, and delivering functional rehabilitation in patients' homes.
The role requires regular travel to patient homes and other care settings. Mileage reimbursement is available for approved travel, and a facility vehicle may also be available.
ResponsibilitiesReview therapy orders, complete comprehensive patient evaluations, and establish individualized treatment plans with short- and long-term goals.
Provide skilled physical therapy interventions, including therapeutic exercise, stretching, mobility training, balance activities, pain management, and functional rehabilitation.
Assess home safety and recommend appropriate equipment, assistive devices, and environmental modifications to improve independence and reduce fall risk.
Monitor patient progress, modify treatment plans as needed, and educate patients, caregivers, and family members on home exercise programs and safety measures.
Maintain timely clinical documentation and collaborate with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
Minimum 2 Years of Physical Therapist Experience Required.
Current, Active Montana Physical Therapist License Required.
Current BLS Certification Required.
Valid Driver's License Required.
Ability to Travel to Patient Homes and Other Assigned Care Settings and Provide Independent Home-Based Physical Therapy Services Required.
