About the RoleWe are seeking an experienced Physical Therapist (PT) Home Health for a 13-week contract in Rutland, Vermont. The Physical Therapist will provide comprehensive home-based rehabilitation services, including patient assessments, individualized treatment planning, skilled therapy interventions, patient and caregiver education, and coordination of care with the interdisciplinary healthcare team.
The PT will also complete OASIS assessments and required clinical documentation, manage patient visit schedules, coordinate safe discharges and transfers, and supervise appropriate rehabilitation support personnel in accordance with applicable standards and regulations.
ResponsibilitiesPerform comprehensive Physical Therapy and OASIS assessments and develop individualized plans of care based on patient needs, functional goals, and safety considerations.
Provide skilled physical therapy treatments, therapeutic interventions, and patient/caregiver education while monitoring progress during each visit.
Coordinate patient care with physicians, nurses, caregivers, rehabilitation professionals, and other members of the interdisciplinary healthcare team.
Complete accurate and timely clinical documentation, OASIS time points, insurance-related forms, and other required home health records.
Manage patient schedules, coordinate safe discharges or transfers, and supervise PTAs, students, and home care aides as applicable.
Minimum 2 Years of Physical Therapist Experience Required.
Home Health Physical Therapy Experience and Ability to Complete OASIS Assessments Required.
Current Physical Therapist Licensure Required Upon Acceptance; Applicable State Licensure Requirements Must Be Met Before Start.
Ability to Work Monday Friday, 8:00 AM 4:00 PM, Including 2 3 Required Weekend Days and Holiday Coverage.
Experience With Home Health Documentation and Care Coordination Required; McKesson Experience Preferred.
