This role focuses on improving patients’ independence, safety, and overall quality of life through individualized treatment plans and collaborative care.
Essential Responsibilities
- Evaluate patients to identify speech, language, cognitive, communication, and swallowing deficits.
- Develop individualized treatment plans based on physician orders and patient goals.
- Provide therapeutic interventions to improve communication abilities, cognitive functioning, and swallowing safety.
- Educate patients, families, and caregivers on treatment strategies and home exercise programs.
- Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.
- Maintain accurate and timely clinical documentation in accordance with Medicare and agency guidelines.
- Participate in care conferences and contribute to patient care planning.
- Ensure compliance with all agency policies, state regulations, and professional standards.
