Job Overview
We are seeking a compassionate Home Health Registered Nurse (RN) to provide nursing care to patients in their homes. The RN will coordinate care with the interdisciplinary team, patients, families, physicians, and community partners to ensure patients are receiving the care they need. This position requires strong clinical judgment, excellent communication skills, and a passion for home health care.
Responsibilities
- Perform initial and ongoing comprehensive assessments of home health patients
- Determine home health eligibility and appropriate levels of care per Medicare and agency guidelines
- Develop, implement, and revise individualized plans of care
- Educate patients and families on disease progression, symptom management, and treatment plans
- Coordinate care with physicians and attend case conference
- Supervise and provide clinical direction to Aides and LPN/LVNs
- Monitor caregiver capability and provide education and support
- Complete accurate, timely clinical documentation and progress notes
- Communicate changes in patient condition to physicians and care team
- Follow infection control and standard precautions
- Promote agency policies, philosophy, and compliance with all regulations
Minimum Qualifications
- Graduate of an accredited School of Nursing (Diploma, ADN, or BSN)
- Current, unrestricted Oklahoma RN license
- Current CPR certification
- Valid Oklahoma driver’s license
- Reliable transportation and current auto liability insurance
- 2 years of clinical nursing experience required
- Hospice, home health, or palliative care experience preferred
Pay: From $55.00 per hour
