The office includes:
- Four experienced family medicine physicians
- A collaborative and supportive clinical team
- Dedicated medical assistants and patient service representatives
- An established patient population and strong community reputation
- Monday - Thursday: 7:00 AM – 4:00 PM
- Friday: 7:00 AM – 3:30 PM
- Perform comprehensive patient evaluations including medical histories, physical examinations, and diagnostic assessments
- Diagnose and manage acute and chronic medical conditions across the lifespan
- Provide preventive care services including annual wellness visits, screenings, immunizations, and health counseling
- Develop and manage treatment plans for chronic diseases such as diabetes, hypertension, asthma, COPD, and hyperlipidemia
- Prescribe medications and monitor treatment effectiveness in accordance with Ohio licensure and practice guidelines
- Educate patients and families on disease prevention, treatment options, medications, and healthy lifestyle strategies
- Collaborate with physicians, nursing staff, and specialists to coordinate comprehensive patient care
- Facilitate referrals, follow-up care, and transitions of care as needed
- Maintain accurate and timely documentation within the electronic health record system (EPIC)
- Master’s degree from an accredited Nurse Practitioner or Physician Assistant program
- Current Ohio licensure as a Nurse Practitioner or Physician Assistant
- National certification as a CNP or PA-C
- Minimum of one year of post-graduate clinical experience preferred, ideally in outpatient primary care or family medicine
- Strong clinical assessment, diagnostic, and patient management skills
- Excellent communication and interpersonal abilities
- Ability to work independently while collaborating effectively within a multidisciplinary team
- Experience with EPIC EHR preferred, though training will be provided
