COMPENSATION

  • Unlicensed Therapist Target Range - $55,000-$65,000 non-exempt (based on clinical hours and supervision)
  • Licensed Therapist Target Range: $82,500 annual exempt

 

SCHEDULE

Monday–Friday, 7:30 a.m.–3:30 p.m.

Rotating weeks of after-hours/weekend clinical coverage as part of the ACT team

Central Minnesota Mental Health Center (CMMHC) is looking for an ACT Assistant Team Lead to join our St. Cloud team. ACT is hands-on, community-based, team-driven work—and no two days look the same.

 

One day you might be providing therapy in a client’s home. The next, you could be helping someone navigate a housing challenge, coordinating care with another provider, supporting medication management, or working alongside your multidisciplinary team to respond to a change in someone’s mental health.

You won’t spend your days sitting behind a desk waiting for clients to walk through the door.  You’ll meet people where they are, literally and figuratively, and help them build lives in the community they call home.

 

What Makes ACT Different?

Assertive Community Treatment (ACT) is an intensive, community-based mental health service for people living with serious mental illness who may face multiple barriers to stability and independence, including substance use, homelessness, unemployment, and other complex challenges.

 

ACT takes a “whatever it takes” approach to helping people succeed.

  • Work happens in the community. Services are provided in clients’ homes and other community settings—not just in an office.
  • Every day is different. ACT clients have complex and changing needs, so the work is dynamic, creative, and rarely predictable.
  • You’re part of a team. Therapists, nurses, psychiatrists, rehabilitation specialists, and other professionals work together toward shared goals.
  • You get to be hands-on. You’re not just talking about solutions—you’re helping clients put those solutions into action.
  • Relationships matter. ACT is built around consistent, long-term relationships and meeting people where they are.
  • You can see the impact. Whether it’s helping someone find housing, reconnect with treatment, manage symptoms, or regain independence, your work can have a tangible impact on someone’s life.

 

ACT services are provided by a multidisciplinary team and combine treatment, rehabilitation, therapeutic, and supportive services. Services are intensive, available on a time-unlimited basis, and designed to support individuals in living successfully in the community of their choice.

 

The Opportunity

As the ACT Assistant Team Lead, you’ll play a key role in the clinical leadership and day-to-day success of the team.

 

Working within a shared leadership model alongside the ACT Team Lead and team psychiatrist, you’ll help ensure the team meets ACT standards as outlined in the Tool for Measurement of ACT (TMACT), while supporting high-quality services for clients and a strong, collaborative team environment.

 

This is a role for someone who enjoys both leadership and direct clinical work—someone who can step back and see the big picture while also being willing to roll up their sleeves and jump in when the team or a client needs support.

 

What You’ll Do

  • Support compliance with ACT standards outlined in the Tool for Measurement of ACT (TMACT).
  • Provide direction, clinical support, and leadership to team members to promote quality and stability in daily operations.
  • Assume clinical oversight when delegated or when the Team Lead is unavailable.
  • Provide individual, group, and family therapy when clinically indicated.
  • Provide services in clients’ homes and throughout the community.
  • Adapt Individual Treatment Plans as clients’ mental health symptoms, behaviors, and needs change.
  • Complete Diagnostic Assessments and contribute to case formulation and treatment planning.
  • Coordinate care across CMMHC programs and with outside providers and community partners.
  • Assist with referrals and screenings.
  • Under the direction of the psychiatrist and Registered Nurse, provide medication monitoring and health education, support medication adherence, deliver medications when appropriate, and communicate medication concerns to the RN.
  • Participate in required team meetings and other mandatory meetings.
  • Help the team problem-solve complex situations and identify creative ways to support clients in achieving their goals.
