CMMHC has a rewarding opportunity for a Mental Health Professional or Clinical Trainee to serve as Team Lead for our St. Cloud Assertive Community Treatment (ACT) team.

 

As the ACT Team Lead, you'll be at the center of that work. You'll provide clinical leadership to a collaborative team while also staying connected to the people we serve and the realities of community-based care. You'll help the team navigate complex situations, coordinate across systems, respond to crises, and turn the ACT model into meaningful, person-centered support.

 

ACT is different from traditional therapy—and so is this role.

 

Rather than providing therapy from an office or managing an individual caseload, ACT brings a multidisciplinary team into the community to support people with serious and persistent mental health needs. Team members work together, share responsibility for clients, respond to changing needs, and help people build stability and independence in the places where they live and work.

COMPENSATION

Target Range – $75,000 - $90,000 (Commensurate with experience)

 

SCHEDULE

Monday-Friday, 7:30 am-3:30 pm

Rotating weeks of after-hours/weekends clinical coverage as part of the ACT team

 

 

HOW YOU'LL MAKE AN IMPACT

 

As ACT Team Lead, you'll help ensure the team delivers services consistent with the PACT Model, Minnesota Statutes, the Tool for Measurement of Assertive Community Treatment (TMACT), and other evidence-based ACT practices.

 

Your leadership will help the team:

  • Lead as a team, not in silos. Promote shared responsibility, strong communication, and collaborative decision-making across the multidisciplinary ACT team.
  • Meet people where they are. Support community-based services in clients' homes, neighborhoods, and other community settings rather than relying solely on traditional office-based care.
  • Respond to complex and changing needs. Provide clinical leadership as the team responds to psychiatric symptoms, crises, hospitalizations, housing instability, substance use, medical concerns, and other challenges that can impact a person's ability to remain successful in the community.
  • Keep recovery at the center. Help clients build greater stability, independence, meaningful relationships, employment, housing, and community connections while respecting their goals, choices, and autonomy.
  • Coordinate the whole picture. Assess, refer, coordinate, and monitor services while communicating with providers and community partners to reduce duplication, close gaps in care, and ensure clients receive the support they need.
  • Bring the right resources to the table. Maintain current knowledge of mental health, medical, housing, employment, social service, and community resources and help the team connect clients with those resources.
  • Provide clinical leadership beyond 9-to-5. Maintain overall clinical oversight of the ACT team after regular business hours, on weekends, and holidays. This responsibility may be delegated to another qualified ACT team member when appropriate.
  • Build a strong team. Maintain optimal staffing levels, participate in hiring, oversee staff training, provide coaching, and conduct performance evaluations while addressing performance concerns in a timely and supportive manner.
  • Lead program operations. Support the financial viability of the program and provide the planning and operational leadership necessary to secure the human, financial, and technology resources needed for the team to succeed.
