Central Kansas Mental Health Center has an opening for a Care Coordinator. The Care Coordinator is a position that will be responsible for coordination of care between persons receiving services and their service providers within the Certified Community Behavioral Health Clinic (CCBHC) and in the community.
We want to provide a holistic and person-centered approach to the care we provide which can also include meeting persons receiving services in the community to assist in bridging barriers they may have to receiving their services.
We are looking for someone who can be personable and professional with a wide range of people with different backgrounds, accurately recording data, and assist our individuals and their families with other services as directed by their treatment plans.
