The Partnership Specialist is responsible for maintaining and strengthening relationships with existing organizational partners across the post-acute and ambulatory care continuum. This role focuses on relationship growth, partner engagement, and operational alignment with facilities that already work with the organization.
The position does not involve lead generation, sales prospecting, or new market development. Instead, the focus is on ensuring current partners remain informed, aligned, and connected to the broader care network supported by the organization.
This role acts as a relationship liaison between partner facilities and the company’s physician services, ensuring seamless communication, awareness of services, and alignment with referral pathways.
Candidates must be in the Orlando area with the ability to travel to local hospitals/facilities to build relationships.
Salary range: $55-65K, based on experience and skillset
Core Responsibilities
Relationship Management
- Maintain and strengthen relationships with existing partner facilities and healthcare organizations.
- Serve as the primary liaison between partners and internal clinical leadership.
- Conduct routine visits, check-ins, and engagement activities with existing customers.
- Support alignment across the continuum to improve care transitions and communication between organizations.
- Facilitate collaboration between existing partners and other customers within the network, including:
- Skilled Nursing Facilities (SNF)
- Assisted Living Communities (ALF)
- Hospitals
- Inpatient Rehabilitation Facilities (IPR)
- Long-Term Acute Care Hospitals (LTAC)
- Primary Care Physicians (PCP)
Partner Education & Updates
- Ensure partner facilities remain informed about:
- Organizational services and capabilities
- Provider coverage and physician alignments
- Attestation processes
- Referral alignment opportunities
- Operational updates that impact partner facilities
Communication & Engagement
- Provide updates to facilities regarding physician services, rounding schedules, and care coordination initiatives.
- Maintain strong communication with hospital discharge planners, facility leadership, and physician teams to ensure operational clarity.
- Actively represent the organization at community events, industry functions, and events hosted by prospective and existing referral partners to strengthen relationships, increase brand visibility, and generate new business opportunities.
- Follow Progress Reporting system to document activity.
Operational Support
- Assist partner facilities with understanding referral pathways and provider alignment.
- Support documentation or communication initiatives related to partner engagement.
- Ensure internal leadership is aware of partner feedback, operational needs, and opportunities for service improvement.
Qualifications
Required
- Bachelor’s degree in healthcare administration, business, marketing, or related field (preferred but not required with experience)
- Experience in healthcare relationship management
