What are the responsibilities and job description for the Client Care Coordinator position at Central Arizona Aging?
Job Summary
Responsible for coordinating client care in Pinal and Gila Counties and understanding the diverse population and needs of the areas served by Central Arizona Aging (CAA). This position also entails working closely with the Independent Living Services Director to develop client coordinated programs based on the requirements of contracts.
Duties
- Serves as the direct liaison in representing CAA to other agencies, stakeholders, and all other interested parties. Raise awareness in the community of CAA services and advocates for the aging population.
- Willingness to adapt to changing client needs and work within healthcare settings.
- Liaison with community organizations and maintaining good relationships with stakeholders.
- Prepares all required documentation and reports required of the position.
- Ability to create care plans based on assessments from referral source.
- Manage and maintain confidential client records and administrative documentation related to care coordination.
- Designs targeted outreach programs, including, but not limited to, attending community meetings that are relevant to the client population, coalition meetings, network meetings, etc.
- Provide education to the community on CAA services and other pertinent age-related topics.
- Have the knowledge and resources management to connect support group members with local resources and support services.
- Strong verbal and written communication skills are essential for effectively interacting with clients, families, other teams, and healthcare systems.
- Attends agency, department meetings, events, workshops, and training.
- Understand and manage budgets.
Qualifications
- A bachelor's degree from an accredited college in a field related to case management such as social work, nursing, public health or other human services related field. Comparable professional knowledge, skills, and abilities that document 4 years experience specific to case management may be substituted for the degree. Must have a minimum of two (2) years experience in serving the community. Possess a current AZ Driver's License and 36 month driving record. Current Level 1 Fingerprint Clearance Card and must pass Central Registry Background Check.
- Knowledge of the community and the diverse population within the counties served. Possess effective public speaking skills, initiative-taking, detail oriented, excellent communicator, strong interpersonal skills, good organizational skills, and work independently. Prepare monthly records and reports. Good verbal and written skills. Ability to collaborate with individuals of various educations. cultural, and social backgrounds. Ability to type and knowledge of Microsoft Office.
Pay: $19.06 - $20.84 per hour
Expected hours: 40 per week
