Manager Home Care is responsible for the development, implementation, coordination, and management of home-based personal care services for PACE program participants. This position will provide direct and indirect supervision to all home care certified nursing assistants. This position acts as the leader in collaborating with members of the interdisciplinary team, providing the foundation for creating the interdisciplinary plan of care. He/She works to promote and sustain a culture reflective of Centra's best practices for quality and service, creates and maintains a safe home care environment, and contributes to a healthy work environment.


The Home Care Manager is responsible for the development, implementation, coordination, and overall management of home-based personal care services for PACE program participants. This role provides both direct and indirect supervision of all home care Certified Nursing Assistants (CNAs) and serves as the supervising nurse under whose licensure CNAs practice. The Home Care Manager plays a key leadership role within the leadership and interdisciplinary teams, supporting the development and execution of comprehensive, participant-centered plans of care.


Required Qualifications:

  • Graduate of an accredited nursing program with a current Virginia or multistate nursing license
  • Two (2) years of nursing experience with at least two (2) years of supervisory experience and one (1) year of caring for the frail or elderly.
  • Hold a current, active, American Heart Association Basic Life Support (AHA BLS) course completion card or to be obtained on hire
  • Bachelor of Science in Nursing, or to be obtained within 24 months

Preferred Qualifications:

  • Master’s degree in nursing, Healthcare, or business
  • Two (2) years of nursing experience in home health or long-term care.

Essential Duties and Responsibilities:


  • Assesses the home care needs of frail elderly participants and develops specific plans of care to address identified needs
  • Participates in the development of the participant plans of care, the coordination of a 24-hour care delivery system, and the interdisciplinary team (IDT) plan of care
  • Coordinates the implementation of home care services, including personal care, homecare-chore services, and ensures quality services are provided according to the needs of individual participants
  • Supervises all contracted care with outside CNA/PCA agencies, if utilized, to include orienting, scheduling, audits, drafting and documenting all plans of care to be implemented, conducting supervisory visits and working with the business office to ensure billing is correct.
  • Serves as a coach, educator, and resource to home care certified nursing assistant staff by performing joint home visits to promote development of staff
  • Counsels home care certified nursing assistants when expectations and policies are not being followed correctly. Provides follow up to staff after counseling. Documents in employee files appropriately
  • Performs human resource management of all home care staff
  • Uses established processes and electronic systems to approve requests for compensation and expense reimbursement
  • Provides appropriate instruction and direction to home care certified nursing assistant staff
  • Participates in the PACE management team and serves as liaison between administration and in home certified nursing assistants
  • Can conduct initial and regulatory assessments of participant needs
  • Communicates participant changes to team members
  • Completes all documentation in the medical record as required
  • Investigates complaints and incidents in a timely manner and resolves issues, as able
  • Nursing supervision visits in participants home at least every 90 days and with concerns raised from the patient, caregiver, or PACE team members.
  • Direct oversight of PACE home care budget in collaboration with PACE administrative team.
  • Develop, implement, coordinate, and manage home-based personal care services for PACE participants
  • Provide direct and indirect supervision, guidance, and performance oversight of home care CAN
  • Serve as the supervising nurse, ensuring CNAs practice in compliance with licensure, regulatory, and organizational standards
  • Collaborate with the interdisciplinary team to support the creation, implementation, and evaluation of individualized plans of care
  • Promote and sustain a culture aligned with Centra’s best practices for quality, service excellence, and participant satisfaction
  • Ensure a safe, compliant, and supportive home care environment for participants and staff
  • Support staff development, coaching, and accountability to maintain high-quality care delivery
  • Contribute to a positive, healthy, and respectful work environment
  • Monitor care delivery to ensure regulatory compliance, quality outcomes, and continuous improvement. This is to include in-home supervisory visits alongside the CNAs every 90 days and/or as needed if concerns are raised.
Leadership & Collaboration
  • Act as a leader and key collaborator within the PACE leadership team, as well as the interdisciplinary teams at each site
  • Foster strong communication between CNAs, nursing staff, and other care team members
  • Support organizational goals related to quality, safety, budget, and service excellence
Other Functions:
  • May perform other duties as assigned or requested and job specification can be modified or updated at any time

Supervisory Responsibilities:
This role directly supervises three health unit coordinators- Unit Support Associates (USAs) as well as 25 home certified nursing assistants (CNAs)
