Social Worker | Monticello Hospital

Find your purpose as a Social Worker, in an essential role, supporting patients and their families while in the hospital! As an integral part of CentraCare Health – Monticello, the Social Worker will collaborate with the social work team supporting all units at the Monticello hospital including the Geriatric Behavioral Health Unit (GBHU), Hospital Inpatient, and Emergency Room. The social worker provides care to facilitate achievement of expected patient outcomes through a collaborative process of assessment, planning, care coordination, communication and advocacy for options and services to meet individuals' needs. Social workers will ensure compliance with licensing boards, professional standards and accrediting agencies while providing effective, efficient services and ensure appropriate use of resources across the continuum. Social Workers will promote patient/resident and family centered care to achieve CentraCare Health – Monticello's mission working to improve the health of every patient, everyday. This role demands strong communication, empathy, and problem-solving skills to support patients through their healthcare journey effectively. Schedule

• Full-time- 72 hours every two weeks- your scheduled day off every pay period may vary.
• Working Days are Monday through Friday
Shifts are 8:00am-4:30pm primarily, however can also vary depending on need.

Pay and Benefits

• Starting pay begins at $63,752.00 and increases with experience
• Salary range: $63,752.00-$95,638.40 per year
  • Salary range is based on a 1.0 FTE, reduced FTE will result in a prorated offer rate
• $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates!

Qualifications

• Current Licensed Social Worker (LSW) in Minnesota required, OR current Licensed Graduate Social Worker (LGSW) required OR Licensed Independent Social Worker (LICSW) required.
• Bachelor's degree in social work.
• At least two years of experience as a Social Worker- required, preferably in a healthcare setting.
• Knowledge of Joint Commission, MN Department of Health standards and various other regulatory standards.
• Ability to read and communicate effectively in English.
• Strong written and verbal skills.
Strong interpersonal human relations skills.

To be eligible for the hiring bonus you must be an external candidate who is hired into an eligible full time or part time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.

Relocation Assistance Eligibility: Must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. Must currently reside more than 50 miles away from CentraCare Monticello Hospital and relocate to within 30 miles of it.
