As a Medical Social Services Professional, you'll play a critical role in assessing patients' emotional and social needs, identifying and connecting them with community resources, and providing education and support to help them manage their chronic conditions and navigate the healthcare system.

Essential Responsibilities
- Assess patients' emotional and social needs, developing personalized plans of care to address these needs
- Work collaboratively with the care team to ensure that patients receive the highest quality care
- Identify and connect patients with community resources to support their recovery and well-being
- Provide education and support to patients and families on managing chronic conditions and navigating the healthcare system
- Collaborate with physicians and other healthcare professionals to ensure that patients receive coordinated and comprehensive care
Requirements
- Master's or doctoral degree in social work from an accredited program
- Licensure as a social worker in the state of practice (if required by state law or regulation)
- Experience working in a healthcare setting, preferably in home health or hospice
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
- Ability to prioritize tasks, manage multiple responsibilities, and maintain accurate records
- Valid driver's license and reliable transportation (for visiting patients in their homes)
