The Lead Client Care Coordinator coordinates all aspects of direct care including intake, start of care and ongoing service needs. Ensures quality of care by acting as a liaison between clients and Client Care Team, communicating staffing, training and equipment needs. Provides in-the-field support for both clients and Home Health Aides.
