Fairview Skilled Nursing Home is seeking an experienced MDS Coordinator RN to oversee the MDS/RAI process, ensure timely and accurate assessments, maintain regulatory compliance, and support quality resident care and reimbursement.
Responsibilities
- Complete and coordinate MDS, CAAs, and care plans.
- Ensure compliance with CMS, Medicare, Medicaid, PDPM, and OBRA regulations.
- Monitor assessment schedules and submission deadlines.
- Collaborate with the interdisciplinary team on resident assessments and care planning.
- Review documentation for coding accuracy and reimbursement.
- Monitor Quality Measures and assist with QAPI initiatives.
- Support survey readiness and educate staff on MDS documentation.
- Current New York State RN license.
- MDS Coordinator experience in a skilled nursing facility preferred.
- Knowledge of MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.
- Strong organizational, communication, and documentation skills.
Salary : $35 - $55
