Fairview Skilled Nursing Home is seeking a detail-oriented MDS Assessor RN to complete resident assessments, ensure accurate MDS documentation, support regulatory compliance, and contribute to quality resident care.
Responsibilities
- Complete MDS assessments accurately and on time.
- Gather assessment information from residents, families, and the interdisciplinary team.
- Ensure documentation supports MDS coding and regulatory requirements.
- Collaborate with nursing, therapy, social work, and other departments.
- Maintain compliance with CMS, Medicare, Medicaid, and PDPM guidelines.
- Assist with care planning and quality improvement initiatives.
- Current New York State RN license.
- Skilled nursing or long-term care experience preferred.
- Knowledge of MDS 3.0, PDPM, Medicare, and Medicaid.
- Strong assessment, organizational, and communication skills.
Salary : $35 - $55
