What are the responsibilities and job description for the Per Diem Case Manager position at CENTER FOR URBAN COMMUNITY SVC?
The Center for Urban Community Services, a national leader in the development of effective housing and service initiatives for homeless people, invites applications for a Case Manager’s position at its newest permanent supportive housing program, Williamsbridge Gardens. The program provides 170 units of permanent housing for both low income and formerly homeless singles and families, many of whom have histories of mental illness and substance abuse. Williamsbridge Gardens is in the Williamsbridge Gardens section of the Bronx, within walking distance of multiple modes of public transportation.
CUCS provides comprehensive on-site social services using a recovery orientation, which includes evidence-based practices with particular emphasis on mental health and substance abuse recovery for singles, and a family systems approach for the families.
Responsibilities: Work with per diem social worker to provide a wide range of clinical services, including but not limited to: Wellness outreach to tenants; supportive counseling; facilitating organized activities; crisis intervention; assistance with self administration of medications; community escorts; coordination with hospitals/collateral providers and a variety of other case management tasks, as needed. The per diem case manager will document all client interactions in progress notes and a weekend program log to keep full time staff informed of tenant updates.
Schedule: Friday evenings 4:30pm – 9pm & Saturday 10am – 4pm, alternate weekends. Friday arrival time can be negotiated for otherwise strong candidates. As needed, weeknight and holiday shifts may be offered, as per site coverage needs.
