Center for Neurology and Spine is seeking a Board-Certified Family Nurse Practitioner (FNP-BC) to support patients with complex neurological conditions who benefit from a palliative care–focused approach to symptom management and quality of life.
This role focuses on patient-centered care, symptom control, and care coordination for individuals living with chronic neurological illness.
Conditions Commonly Managed
Patients appropriate for neurological palliative care often include those with:
- Parkinson’s disease
- Multiple sclerosis (MS)
- ALS
- Dementia
- Stroke complications
- Chronic neuropathic pain
- Traumatic brain injury
- Advanced epilepsy
These patients require ongoing symptom management, supportive care, and interdisciplinary coordination.
Responsibilities
The FNP-BC will conduct comprehensive evaluations including:
- Comprehensive health assessments
- Symptom burden evaluations
- Medication reconciliation
- Functional status assessments
- Cognitive assessments
- Psychosocial evaluations
The provider will identify and manage symptoms such as:
- Pain
- Fatigue
- Anxiety
- Depression
- Insomnia
- Appetite changes
- Dyspnea
- Neuropathic discomfort
Additional responsibilities include care coordination with neurologists, primary care providers, and families to support optimal quality of life.
Qualifications
- Family Nurse Practitioner – Board Certified (FNP-BC)
- Active or eligible Arizona NP license
- DEA license or ability to obtain
- Experience in neurology, geriatrics, palliative care, or chronic disease management preferred
- Strong patient communication and care coordination skills
Position Details
Location: Phoenix, AZ
Schedule: Contract Preferred
Setting: Outpatient neurology practice, or patient home visits.

Compensation
- Pay is based upon the number of patient visits and the type of visit being performed.
- An hourly rate is reflected above, which is based upon the expected number and type of visits performed.
Pay: $50.00 - $65.00 per hour
Education:
- Master's (Required)
Experience:
- FNP-BC: 3 years (Required)
Willingness to travel:
- 25% (Preferred)
