- Develop meaningful, individualized implementation strategies based on each person's Person-Centered Plan.
- Provide transportation as needed to support community networking goals.
- Document progress in Therap within 24 hours of each activity record completion.
- Ensure services align with personal outcomes in each individual's support plan.
- Plan cost-effective travel routes and manage caseload efficiently.
- Provide companionship, emotional support, and behavioral crisis intervention as needed.
- Complete all required training and certifications before expiration.
Required
- High school diploma or GED.
- Valid driver's license, clean driving record, and proof of insurance.
- Must be at least 18 years old and able to pass background and registry screening.
- Ability to obtain CPR and First Aid certification.
- Familiarity with community-based services for individuals with IDD.
- At least one year of experience supporting community integration for individuals with disabilities.
- Pay: $19.00/hour mileage reimbursement
- Schedule: Full-time | Hours vary based on individuals' availability and preferences
- Work Setting: Individuals' homes and community settings across the Kansas City metro
- Travel: Regular travel throughout the metro area
