We have open positions in Beaver County! Applicants must live in or around Beaver County and be willing to drive throughout the county and meet with individuals face-to-face to complete Supports Coordination activities.
Supports Coordinator Minimum Requirements
- Bachelor’s degree with successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services.
- At least one year’s experience working with individuals with disabilities preferred.
- ACT 33/34 and FBI Clearances, valid driver’s license and proof of car insurance coverage must be provided prior to hire.
- Must maintain strict confidentiality and be able to exercise sound judgement, be well organized and have excellent time management skills.
Supports Coordinator Responsibilities
- Use a person-centered approach to develop and maintain the individual support plan.
- Inform the individual of service and provider choice, assist in the selection of qualified providers.
- Coordinate community living supports that may include but is not limited to residential, vocational, employment, educational, and social organizations.
- Offer community activities and support to individuals as well as resources and support to families.
- Evaluate individual’s progress towards their identified goals and satisfaction of services.
- Respond to incidents to ensure appropriate actions are taken to protect the well-being, health, safety, and rights of the individual.

Pay: $42,000 - $43,000
Schedule

- M-F, no weekends

Education
Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social services.
OR
Two years’ experience as a County Social Service Aide 3 and two years of college level coursework, which includes at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services.
OR
Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services AND one-year experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.
Experience
One-year experience working with individuals with disabilities.
Licenses, Certificates or Clearances
Act 33/34 and FBI Clearances.
Valid Driver’s License.
Car Insurance Coverage Verification.
Special Skills, Knowledge and Abilities
Computer competency with ability to learn agency software.
Knowledge of Community Resources.
