Provide supports coordination services to individuals who meet criteria under the Office of Developmental Programs regulations and bulletins.

Normal office setting with frequent computer work approximately 50% of the time.
Travel to community sites, agencies, and consumer homes with varying degrees of temperature, humidity, and noise approximately 50% of the time in a variety of terrains and weather conditions.
Responsibilities occasionally may require an adjusted work schedule and additional hours worked in order to meet deadlines.
Possible exposure to unsanitary conditions, communicable diseases, cigarette smoke, and animals.

Lifting up to 20 lbs. maximum and occasionally carrying such articles as a laptop, computer, files, ledgers and small tools.

Education
Bachelor’s degree, which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice or other related social services.
OR
Two years’ experience as a County Social Service Aide 3 and two years of college level coursework, which includes at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services.
OR
Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social services AND one-year experience as a County Social Services Aide 3 or similar position performing paraprofessional case management functions.
Experience
One-year experience working with individuals with disabilities.
Licenses, Certificates or Clearances
Act 33/34 and FBI Clearances.
Valid Driver’s License.
Car Insurance Coverage Verification.
Special Skills, Knowledge and Abilities
Computer competency with ability to learn agency software.
Knowledge of Community Resources.
Other Factors or Requirements
