Position Summary:
Requires the necessary skills and knowledge to plan, develop, organize, implement, evaluate and direct new admissions in accordance with current state, federal, and local standards as well as established policies and procedures. Ability to establish and maintain effective working relationships with referring sources, and staff. Must be knowledgeable in the inquiry and admissions process, possess basic knowledge and skill in marketings, and could demonstrate effective oral and written communication in English; lift and carry items up to 30 pounds.
Qualifications of Admissions Coordinator:
- High school Diploma
- Additional education in a Healthcare Discipline
- Preferred, Prior experience in Admissions & Marketing in Healthcare Setting
- Valid PA Drivers Licenses
JOB DESCRIPTION:
- Complete daily census roster.
- Update and log daily census. Maintain & balance Resident Days Report.
- Take phone inquiries and greet walk-in inquiries.
- Complete & coordinate referrals from hospital caseworkers.
- Present referrals to admissions committee daily.
- Coordinate admissions to the facility – notify Nursing and other departments.
- Responsible for completion of Nursing Facility New Admission list for UMR.
- Schedule & perform facility tours.
- Responsible for completion of all 401’s.
- Maintain and have available for UMR
- Responsible for sign-ins of the admissions.
- Explain the process and secure signatures of resident/responsible party utilizing packet of forms.
- Responsible for communicating admissions/discharges, changes in level of care, pay status, and room changes to nursing department, business office, and pharmacy.
- Advise resident and families of Medicare, Medical Assistance qualifications.
- Responsible for initiating bed hold notices and follow through of information to appropriate departments.
- Assist Director of Social Services with staff training sessions, e.g., residents’ rights etc.
- Verifying insurance coverage.
- Assist Director of Social Services as needed.
- Complete & maintain business files on all admissions.
- Complete monthly referral logs & summary reports.
- Participate in developing & implementing marketing plan.
- Practicing safe working habits.
