Position Summary
The Program/Case Manager provides trauma-informed, strengths-based case management to program participants and manages the day-to-day coordination of program services. This role maintains program quality, coordinates volunteers and workshops, facilitates monthly multidisciplinary care meetings, and provides participant transportation or workshop instruction coverage when needed. The Program/Case Manager works closely with the House Manager, therapists, volunteers, community partners, and the CEO to promote participant safety, healing, and progress toward independence.
Essential Duties and Responsibilities
Case Management
- Conduct participant intakes, assessments, and develop individualized service plans focused on safety, stabilization, life skills, job readiness, and financial independence.
- Maintain a manageable caseload and provide regular one-on-one check-ins to monitor progress and revise plans as needed.
- Make timely referrals to mental health care, medical services, legal assistance, education and workforce programs, and other community resources.
- Maintain accurate, timely, and confidential case notes in the program database.
Program Coordination
- Manage daily program logistics including participant schedules, workshop planning, and class coordination.
- Identify program gaps, suggest improvements, and assist with implementation of program enhancements.
- Communicate program updates, participant needs, and safety concerns to the CEO and relevant staff in a timely manner.
Care Coordination Meetings
- Schedule and facilitate a monthly care coordination meeting that includes the House Manager, the Program Participant, and the participant’s Therapist.
- Ensure meetings are structured, trauma-informed, and centered on the participant’s voice and goals.
- Document meeting outcomes, action items, and assigned follow-up responsibilities.
- Attend staff meetings and other meetings as scheduled by CEO.
- Facilitate meetings as needed with survivor's advocate for discharge and disciplinary actions. Volunteer & Workshop Support
- Recruit, train, and coordinate volunteers who support workshops and program activities.
- Provide volunteers with clear role descriptions, trauma-informed guidance, and the materials they need to support participants effectively.
- Serve as backup instructor or coordinate temporary instructor coverage when scheduled instructors are unavailable.
Transportation
- Provide transportation for participants when the transport team cannot provide coverage, following agency transportation policies and safety protocols.
- Maintain a valid driver’s license and required driving credentials and report vehicle incidents promptly.
Crisis Response & Safety
- Respond calmly and effectively to participant crises, following established safety and incident response procedures.
- Collaborate with assigned survivor advocates, therapists, leadership, and external partners when additional supports are required.
- Participate in incident debriefs and assist with developing improvement plans.
Communication & Administration
- Maintain professional, respectful communication with participants, staff, volunteers, and community partners.
- Complete required reports, data collection, and program documentation in accordance with organizational and funder requirements.
- Support scheduling, participant onboarding, and other administrative tasks as requested.
Qualifications Required
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field, or equivalent experience.
- Minimum of three years’ experience in case management, program coordination, or direct service with vulnerable populations.
- Demonstrated knowledge of trauma-informed care, crisis intervention, and strengths-based practice.
- Strong written and verbal communication and computer skills, organizational ability, and attention to detail.
- Valid driver’s license, clean driving record, and able to transport participants when needed.
- Ability to pass background check, periodic drug and alcohol tests, and fingerprint clearance as required.
Preferred
- Master’s degree in a related field.
- Experience supervising or coordinating volunteers or staff.
- Bilingual proficiency (English/Spanish or another community language).
- Familiarity with Arizona social service systems and community resource networks.
- Experience delivering workshops, trainings, or group facilitation.
Working Conditions & Physical Requirements
- Primarily onsite work with regular community travel and occasional evening or weekend hours.
- Requires driving, light lifting, standing for workshop facilitation, and sustained interpersonal engagement with individuals who have experienced trauma.
- Must maintain composure and professional boundaries in emotionally intense situations.
