The Health Home Care Coordinator is responsible for providing comprehensive care management services to individuals with complex medical, behavioral health, and social needs. The Care Coordinator works collaboratively with members, healthcare providers, and community-based organizations to develop and implement person-centered care plans that improve health outcomes and overall quality of life.
Key Responsibilities
- Conduct comprehensive assessments to identify members’ medical, behavioral, and social service needs
- Develop, implement, and regularly update individualized, person-centered care plans
- Coordinate and link members to medical, mental health, substance use, and social support services
- Monitor member progress and adjust care plans as needed to ensure goals are met
- Provide crisis intervention and support when necessary
- Maintain regular contact with members through home visits, phone calls, and community visits
- Advocate for members to ensure access to appropriate services and resources
- Collaborate with multidisciplinary teams including healthcare providers, social workers, and community agencies
- Maintain accurate and timely documentation in compliance with Health Home and regulatory standards
- Assist members with benefits enrollment (e.g., Medicaid, SNAP, housing support)
- Support care transitions, including hospital discharge planning and follow-up care
- Participate in team meetings, case conferences, and supervision
Qualifications:
- Must speak Bengali.
- Bachelor’s degree in Social Work, Psychology, Human Services, Nursing, or a related field required
- Minimum of 2–3 years of experience in care coordination, case management, or social services
- Experience working with high-need populations (e.g., individuals with chronic illness, mental health conditions, substance use disorders)

Pay: $23.00 - $25.00 per hour
