A Care Coordinator Supervisor oversees a team of care coordinators who support members with access to healthcare and social services. This role ensures high-quality care management, compliance with regulations, and efficient coordination of services to improve outcomes.
Duties and Responsibilities:
- Supervise, train, and support care coordinators
- Conduct performance evaluations
- Assigning and managing care coordinator caseloads to ensure equitable distribution and efficient service delivery.
- Monitoring and evaluating care coordinator performance, including reviewing case notes, care plans, and incident reports
- Provide guidance on complex member cases
- Ensure care plans are accurate, updated, and person-centered
- Monitor coordination of medical, behavioral, and social services
- Review documentation for quality and compliance
- Step in on high-risk or escalated cases
- Ensure adherence to policies, state/federal regulations (e.g., Medicaid guidelines)
- Ensuring compliance with regulatory requirements, including LHH (Lead Health Home), CMA (Community Management Agency), and DOH (Department of Health) standards
- Conduct audits of case files and documentation
- Work with healthcare providers, hospitals, and community organizations
- Lead team meetings and case conferences
- Improve care coordination processes and workflows
- Support training and onboarding of new staff
Education:
- Bachelor’s degree required (Social Work, Psychology, Public Health, Nursing, or related field)
- Master’s degree preferred (e.g., MSW, MPA, or Psychology) Experience
- 5 years in care coordination, case management, or social services
- 3 years in a leadership or supervisory role
Skills:
- Must speak Bengali
- Leadership and team management
- Knowledge of CORE/FCM services
- Experience handling a team of 15-20 coordinators
