Case Manager (FT/Mon-Fri 9a-5p)

Description

The case manager is responsible for coordinating services for recovery, employment, education, support groups and housing.

Essential Duties and Responsibilities

• Responsible for the case management of assigned individuals.
•  Makes referrals and assists with the coordination of treatment services.
•  Helps individuals identify community agencies, housing opportunities, resources, and supports.
•  Provides assistance to residents with financial linkages and resource management
•  Coordinates educational and/or vocational skill development.
• Helps each individual develop an informal support system, including natural supports and those requested by the individual.
• Ensure smooth transitions and minimizes fragmentation of services.
• Monitors, tracks, and provides support to the individual.
• Organizes and maintains case records.
• Assists with intake and admission.
•  Serves on various agency committees.
• Assists in the implementation of the agency’s policies and procedures.
• Must abide by the ethical guidelines of the field and any licensing authorities.
• Facilitates the resident government’s program guidelines.
• Carries out duties essential to the above stated responsibilities as assigned.
• Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners.
• Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.
• Regular attendance at work location.

Requirements

•  Enrollment in or graduate of a college degree program in a Human Services related field, with at least 6 months’ experience (may include internship experience).
• Must possess a valid New York State Driver’s license.
