Description
The case manager is responsible for coordinating services for recovery, employment, education, support groups and housing.
Essential Duties and Responsibilities
- Responsible for the case management of assigned individuals.
- Makes referrals and assists with the coordination of treatment services.
- Helps individuals identify community agencies, housing opportunities, resources, and supports.
- Provides assistance to residents with financial linkages and resource management
- Coordinates educational and/or vocational skill development.
- Helps each individual develop an informal support system, including natural supports and those requested by the individual.
- Ensure smooth transitions and minimizes fragmentation of services.
- Monitors, tracks, and provides support to the individual.
- Organizes and maintains case records.
- Assists with intake and admission.
- Serves on various agency committees.
- Assists in the implementation of the agency’s policies and procedures.
- Must abide by the ethical guidelines of the field and any licensing authorities.
- Facilitates the resident government’s program guidelines.
- Carries out duties essential to the above stated responsibilities as assigned.
- Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners.
- Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.
- Regular attendance at work location.

Requirements
- Enrollment in or graduate of a college degree program in a Human Services related field, with at least 6 months’ experience (may include internship experience).
- Must possess a valid New York State Driver’s license.
