Job Details
Our Hospice Liaison RN’s :
- Reviewing patient referrals, discuss hospice care options with patients and family members, and plan with both the patient and the facility to prepare for hospice care.
- Develops a positive working relationship with doctors, nursing facilities, and hospitals who refer clients to hospice.
- Coordinate discharges
- Execute and follow up on referrals
- Advocate for the patient and family and why they should choose Catholic Health
- Act as a liaison to referral sources on behalf of Catholic Home Care and Good Shepherd
- Identify potential opportunities for market growth and Hospice program expansion
Qualifications:
- Active New York State RN or MSW License
- Excellent interpersonal, marketing and customer service skills
- Two years Hospice/Home Care experience preferred
- Strong computer and EMR experience.
Salary Range
USD $100,000.00 - USD $115,000.00 /Yr.

The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
