General Description
Works in cooperation with the Ontario County Department of Social Services to provide coordination and oversight of homelessness response and direct case management and support of individuals and families identified as homeless in Ontario County. This position will also provide coordination and administer the allocation of transportation grants and services for eligible Ontario County households.
Essential Duties and Responsibilities
Direct Services
- Conduct Temporary Homeless Assessment of new clients- daily.
- Review individual cases, assess situation and make appropriate referrals to contract agencies, community resources and inter agency departments as needed.
- Provide and monitor temporary housing placements as necessary.
- Conduct follow up with clients to assure compliance with eligibility requirements
- Coordinate with T/A workers regarding status eligibility and ongoing requirements.
Community Coordination
- Maintain oversight and partner relationships with contract and community service providers, community resources and service providers.
- Provide outreach in the community to address trends, community needs and to maintain up to date information regarding openings or placement opportunities at community resources.
- Maintain databases and tracking systems that support identification of and response to homelessness in Ontario County.
- Serve as point of contact between the Department of Social Services, community resources, partners and public.
- Maintain statistical information and complete reporting as necessary. Supports Mission of the Agency
- Participate in staff meetings and other CCFL-related meetings/events.
- Participate in Continuous Quality Improvement efforts for the program and for the agency.
- Complete required agency and program reports in a timely manner.
- Complete other duties as assigned necessary to advance the response to homelessness and transportation needs in Cayuga County.
- Perform other related duties as required/requested.

Requirements:Education: Bachelor’s degree in social sciences preferred. Associate’s degree required
Experience: A minimum of two years of experience in human services required Knowledge of poverty, homelessness and community conditions that impact financial stability
Relevant years of experience and education will be considered
Additional Qualifications:
- Ability to analyze and interpret data and to handle problem resolution.
- Possession of a valid NYS Driver’s license and a driver's record considered acceptable by agency and insurance carrier.
- Continuous use of a reliable, registered and insured vehicle.
- Willing to travel and to attend evening meetings
- Ability to prioritize assignments, plan, and complete work projects with minimal direction
- Exceptional time management skills and ability to effectively and meet deadlines
- Excellent verbal/written skills
- Ability to work in the following software programs: Windows, Microsoft Office, and multiple online software platforms as required by funders and partner organizations
- Ability to maintain confidentiality and handle confidential information with discretion
- Ability to demonstrate behaviors and attitudes which support organizational mission, philosophy, and policies
- Ability to work in a cooperative and helpful manner with all individuals
- Willingness and ability to foster agency and program wide cooperation and teamwork through use of positive/constructive communication techniques
